Please fill in the form below and submit with your contact and company details to receive an invoice for payment. Proof of payment to be sent to firstname.lastname@example.org
Note that application will only be finalised once proof of payment is received.
- Fees are payable immediately on receipt of invoice (no VAT applicable).

- For queries as to the exact amount due, contact the secretariat at firstname.lastname@example.org
- In the event that membership is rejected for any reason, the fees will be refunded without delay.
Termination of membership
- Three month’s notice is required in the event that a member wishes to terminate membership.
- Members will be terminated should payments be outstanding for a period of 6 months from due date.
- Notice of such intention should be given to the secretariat in writing to email@example.com
- The member remains liable for payment of the annual fees.
